If you’re seeking occupational therapy support for your child, please complete the New Client Enquiry form below. The information you provide helps us understand your child’s needs, goals, and whether The Little OT Co is the right fit for your family.
The Little OT Co provides clinic-based appointments at our Wynnum clinic, Tuesday to Friday. Please include your preferred appointment days and times, as this helps us find the best possible match when a suitable appointment becomes available.
Once your referral is submitted, it will be reviewed by our team. If a suitable appointment becomes available and The Little OT Co is the right fit for your child’s needs, we will contact you via email to arrange an initial appointment.
Important Information
Submitting a referral does not guarantee an appointment. All referrals are reviewed to determine whether The Little OT Co is the right fit for your child’s needs and our current service availability.
You will receive an automatic confirmation email once your referral has been submitted. We will only contact families when a suitable appointment becomes available.
If we are unable to offer an appointment at this stage, your referral will remain on file for 3 months. After this time, you may need to resubmit the form if you are still seeking OT support.
