Intake Process for New Clients

If you're seeking occupational therapy support for your child, please complete the Expression of Interest form below to join our waitlist. The more detail you provide, the better we can understand your child's needs and determine if our services are the right fit. Please also include your preferred appointment days and times, as this helps us match availability when a spot becomes available.​

1. Submit the Expression of Interest Form

Please complete the online form below. This is the first step in joining our waitlist and allows us to gather key information about your child and preferred appointment times.

2. Join the Waitlist

Once submitted, your child will be placed on our waitlist. You’ll receive an automatic confirmation email letting you know we’ve received your form.​

3. Triage & Review

Our team will review your submission to ensure alignment with our services. We consider availability, appointment preferences, and clinical fit when assessing new enquiries.​

4. Appointment Offer (When Available)

If a suitable appointment becomes available and your child’s needs align with our service offerings, we’ll contact you via email to arrange an initial appointment.

Important Note:

We operate on a 10 week term-based schedule and have limited capacity. If you don’t hear from us, it means no requested times are available at this stage. We’ll keep your expression of interest on file for 3 months. After this period, you may need to resubmit the form if you're still seeking support. After-school and off-site mobile appointments are currently at full capacity for Term 2 2025.
